LUXE BIOLOGIX CONSUMER HEALTH DATA PRIVACY POLICY

This Consumer Health Data Privacy Policy describes how LUXE Biologix, LLC (“LUXE Biologix,” “we,” “us,” or “our”) collects, uses, and shares Consumer Health Data that is subject to applicable consumer-health privacy laws, including the Washington My Health My Data Act.

This policy is separate from the general LUXE Biologix Privacy Policy.

6. SPECIFIC AFFILIATES

LUXE Biologix is operated by LUXE Biologix, LLC.

As of the Last Updated date of this policy, LUXE Biologix does not identify a separate affiliated legal entity with which it routinely shares Consumer Health Data as an affiliate for purposes of the Washington My Health My Data Act.

If this changes in a manner requiring disclosure, this policy will be updated before the additional sharing occurs as required by applicable law.

7. YOUR CONSUMER HEALTH DATA RIGHTS

Subject to applicable law, you may have the right to:

• Confirm whether LUXE Biologix is collecting, sharing, or selling Consumer Health Data concerning you
• Access Consumer Health Data concerning you
• Obtain information concerning third parties and affiliates with whom Consumer Health Data has been shared or sold
• Withdraw consent from certain collection or sharing of Consumer Health Data
• Request deletion of Consumer Health Data concerning you
• Exercise other rights provided by applicable consumer-health privacy law

LUXE Biologix does not knowingly sell Consumer Health Data.

You are not required to create a new account solely to exercise Consumer Health Data rights, although we may ask you to use an existing account where permitted by law.
